For care programs, research sites and clinics

A local navigator for every patient who needs one.

Fanoni runs care navigation for organizations that need patients to stay on track between visits. Our navigators call, remind, book rides and solve everyday problems, using Fanoni’s own care software, and you get a clear report every month.

A named navigator for each enrolled participant: visit reminders, rides, travel questions and an early warning when someone is at risk of dropping out. Your site keeps screening, consent and study data.

How a pilot works

From first call to day 90

  1. 1

    Agree the pilot

We set the patient group, goals, monthly fee and reporting with you, and sign a business associate agreement.

  2. 2

    Share the patient list

You send the list of patients who agreed to take part, through a secure channel. Never through this website.

  3. 3

    Navigators start

Your patients hear from their Fanoni navigator within one week of the list arriving.

  4. 4

    Review at 90 days

You get a scorecard every month. At day 90 we review the results against the goals and decide together what comes next.

Built on our own software

One navigator can do more when nothing is on paper

Our navigators work in Fanoni’s care software, the same platform behind the Fanoni EHR. It lets one person keep track of many patients without dropping anyone, and gives you a record you can audit.

  • Enrollment, consent and patient lists in one place
  • Every call, text and visit logged with the time spent
  • Secure messages with patients and family members
  • Monthly reports drawn from that record, not from memory

Questions from program and site leaders

Who are the navigators?

Trained community health workers and care coordinators employed and supervised by Fanoni. They are not doctors or nurses and do not give medical advice. Clinical questions go to your team.

Where do you work?

We are starting with pilots in Texas. Phone and text support can reach patients in other states. Ask us about in-person coverage in your area before you plan a pilot.

How is it priced?

A flat monthly fee for each enrolled patient, agreed in writing before the pilot starts. We do not take a percentage of your Medicare, Medicaid or insurance payments.

Do you bill Medicare or insurers for our patients?

No. If your practice bills for care management or navigation, your clinicians remain the billing practitioners. We provide the documentation, and your team reviews and submits any claims.

How do you protect patient information?

We sign a business associate agreement before any patient information is shared. Our software runs on AWS with access controls and audit logs. Please do not include patient details in the form below.

What results can we expect?

We do not publish results we have not measured. Each pilot sets its goals in advance, and the monthly scorecard shows exactly how we are doing against them.
